Preparation for On-campus Vaccination (Matsue Campus)

This web page provides the information of Shimane University's on-campus vaccination on Matsue campus for its members scheduled from Monday, July 5, 2021.

What to bring to the site

○ Prevaccination Screening Questionnaire
 ・The questionnaire is available at faculty office, the Student Center and the Student Support Center. Please obtain and fill it out  in advance, and bring to the site on the vaccination date. 
○ Vaccination card (if you have received from local municipality) 
○ Student ID card / Staff ID card
○ Identification such as My Number Card, driver's license, health insurance card, passport, residence card

※ Please wear clothes that make it easy to access your shoulders
※ Vaccination card is sent from the local municipality and necessary to receive vaccination. However, it is possible to get vaccinated on campus without the card. If you receive the card before the 2nd vaccination, please submit it on the site.
 Those who do not receive the card at the time of the 2nd vaccination will be contacted by the university.
